Qualifications:

• Must have an active PRC License
• At least 2 years of case management experience
• Must have a Certified Case Manager (CCM) certification
• Can work onsite

Responsibilities:

• Case Review & Assessment: Conduct comprehensive assessments of patients’ medical history, treatment plans, and healthcare needs.
• Care Coordination: Collaborate with healthcare providers, insurance companies, and patients to develop and implement care plans.
• Utilization Management: Ensure medical services align with health plan policies and compliance standards.
• Telephonic Support: Provide professional guidance and support to patients through inbound and outbound calls.
• Documentation & Reporting: Maintain accurate and detailed patient records, ensuring proper documentation for case reviews.
• Compliance & Quality Assurance: Adhere to healthcare regulations and company protocols to optimize patient care and cost-effectiveness.
